Vantage Medical Associates is a rapidly growing multi-specialty group practice. We are looking to fill a position for a Spanish Speaking Primary Care Nurse Practitioner or Physician Assistant to work from home and perform telemedicine services. This role is completely remote and no in-person work is required. The role is needed to complete telemedicine home care assessments and chronic care management. Full- Time Position. Monday-Friday 9am-5pm or flexible based on candidate.

The selected candidate will be responsible for telemedicine exams for patients receiving home care services. These patients have routine forms and assessments that need to be completed in order for them to continue their services. The role will require assessment and documentation of patient condition. Exams should be completed 5 per hour.

Your responsibilities would include, but not limited to the following:

• Clear and accurate medical note writing
• Ensures fulfillment of Vantage Medical Associates commitment for ensuring quality care
• Diagnoses and treats variety of diseases in general practice
• Collects, records and maintains accurate patient information
• Documents items such as: past medical, family, and social history, review of systems, examinations, medications, allergies, assessment and plan
• Orders or executes various tests, analyses, and diagnostic images to provide information on patient's condition
• Orders lab and imaging tests to determine and manage an immediate treatment plan
• Advises patients concerning diet, hygiene, and methods for prevention of disease
• Consult with appropriate medical specialist when necessary; refer patient to subspecialists within network

The successful candidate will have the following Qualifications:

• Nurse practitioner or physician assistant degree conferred by an accredited college or university required
• New York State Licensure, Board Eligible and/or Certified and required
• Demonstrated clinical excellence

Pay: $80.00 - $90.00 per hour
