About the Facility: This is a hospital work setting, where you will be providing care to the highest level of care for premature and ill newborns, children, and adolescents as designated by The American Academy of Pediatrics in a level 1 Trauma Center and is a teaching hospital. Total staffed beds are 323 inpatient beds - 70 of them dedicated to Neonatal ICU.
RN Case Manager - RESPONSIBILITIES (listed are included but not limited to) - Participates in all phases of the Case Management Program (CMP) and ensure that the CMP meets established case management standards of care.
- Provide nursing expertise about the CM process, including assessment, planning, implementation, coordination, and monitoring. Identify opportunities for CM and identify and integrate local CM processes.
- Develop and implement local strategies using inpatient, outpatient, onsite, and telephonic Case Management.
- Develop and implement tools to support case management, such as those used to for patient identification and patient assessment, clinical practice guidelines, algorithms, CM software, and databases for community resources.
- Integrate CM and utilization management (UM) and integrate nursing case management with social work case management.
- Maintain liaison with appropriate community agencies and organizations.
- Accurately collect and document patient care data.
- Develop treatment plans including preventive, therapeutic, rehabilitative, psychosocial, and clinical interventions to ensure continuity of care toward the goal of optimal wellness.
- Establish mechanisms to ensure proper implementation of the patient treatment plan and follow-up post-discharge with patients.
- Provide appropriate health care instructions to patients and/or caregivers based on identified learning needs.
RN CM REQUIREMENTS - Education: Must have a Master of Science in Nursing degree from an accredited college or university.
- License: Current, full, active, and unrestricted license to practice as a Registered Nurse in the District of Columbia, or willing to obtain a license in the District of Columbia within 24 hours of position offer.
- Experience: Must have seven (7) years clinical RN experience, and five (5) years Case Management Experience.
- Possess one of the following certifications:
- Certified Case Manager (CCM).
- Advanced Certification in Continuity of Care (ACCC).
- Certified Rehabilitation Counselor (CRC).
- American Nurses Credentialing Center Nurse Case Manager (RN-NCM).
