Brooks Blessings Adult Family Home
·The primary role of the Caregiver on Duty (COD) is to keep the residents safe, cared for and engaged in a home setting. The COD is in the Adult Family Home, actively engaged and is attentive to the needs of the residents while also maintaining the Daily Routine. This may require staying on shift until relief arrives and requires that the COD never abandons the residents by leaving the facility without a qualified Brooks Blessings Employee to monitor their need.
Qualifications:
· Must either: a) meet State certification for Certified Nursing Assistant or Home Care Aide training program and competency and have a credential that is current and in good standing; or b) have applied for Nursing Assistant Register and completed Safety and Orientation training prior to placement, and then complete state certification training and pass the state exam within 120 days, and pass the state exam and receive their license within 200 days, as required by state regulations.
· Complete 12hours of continuing education yearly to maintain their caregiver license and improve skills to better serve as a caregiver.
· Have a sympathetic attitude toward the care of all residents.
· Ability to carry out directions, read and write.
· Maturity and ability to deal effectively with the demands of the job.
Lifting Requirements:
Ability to perform the following tasks if necessary:
· Ability to participate in physical activity.
· Ability to work for extended period of time while standing and being involved in physical activity.
· Heavy lifting.
· Ability to do extensive bending, lifting and standing on a regular basis.
Responsibilities:
· Assist all residents with Activities of Daily Living (ADL’s) as needed and noted in the residents Care Plan and Brooks Blessings Daily Routine (DR).
· Prepare and serve meals as directed by the RM and meal plan.
· Perform the daily choirs as directed but the RM and DR to Brooks Blessings standards and inspection by supervisors, as well as keep the home clean and safe for the residents. This includes making beds, doing laundry, vacuuming, sweeping and moping as needed and directed, cleaning the kitchen after meals are served, doing the dishes, cleaning bathrooms and toilets as needed and other household choirs as directed by the RM or DR.
· Dispense medications as directed by RM and the Medication Administration Record (MAR). This requires that the COD is Nurse Delegated and that the COD pays careful attention to written instructions and uses the “Five Rights” method of medication dispensing: the Right Person, the Right Medication, the Right Dose, the Right Time and the Right Route.
· Complete a thorough shift turnover including counting and recording controlled medications as you come on shift and as you leave your shift.
· Engage personally with residents in the implementation of activities as directed. Monitor residents’ engagement and be proactive in engaging residents to let them know we care.
· Documenting ADL’s, behaviors, meals, toileting, activities, and other health monitor data in Synkwise or other electronic charting systems as directed by RM. Communicating with RM or DOC for any changes of condition, incidents or other critical information that should be passed on to the POA, family or medical professionals.
